In-office precautions during the COVID-19 crisis

COVID-19 screening questions:

Do you have a severe cough?

Do you have significant nasal congestion or significant runny nose?

Do you have a fever > 38 Degrees C?

Do you have a persistent headache?

Do you have shortness of breath?

Do you have diarrhea?

Do you have a reduced or lost sense of smell?

Have you come into contact with anyone that has any of the above symptoms in the last two weeks?

Have you come into contact with anyone suspected of having COVID- 19 in the past 2 weeks?

Have you come into contact with anyone diagnosed with COVID-19 in the past 2 weeks?

All patients using the clinic, for any purpose, will answer the COVID-19 screening questions in the 48-72 hours prior to their appointment and at the time of their appointment.  If any questions are answered in the affirmative, the patient’s appointment will be rescheduled.

All staff will answer the COVID-19 screening questions prior to beginning every work day.  If any questions are answered in the affirmative, staff will not work that day and will not return to work until medically cleared to do so.

All patients will be asked to wait to enter the waiting room/front desk area until only two or fewer other patients are waiting inside the clinic.  Upon check-in with the front desk, Dr. Cox or her testing assistant will immediately come to the front desk to walk the patient back.

Please sanitize your hands upon entering and leaving the building. 

Please wear a mask at all times while in the building.  If you need to adjust or change your mask, please sanitize your hands before removing your soiled mask and after applying your new mask. 

Please attempt to maintain a six-foot distance from others while in the clinic. 

Please closely follow all infection control directions given to you by Dr. Cox and her testing assistant. 

Please feel free to wear your own mask and to bring your own hand sanitizer, if you prefer.  If you do not have a mask, you will be provided with a mask to use during your visit and with hand sanitizer to use as directed during the assessment.  

If you are asked to complete questionnaires or to draw or write, and you have not brought your own pen or pencil, you will be provided with a previously unused pen and/or pencil for your sole use that you may keep or discard at the end of the visit. 

All treatment rooms and all testing materials are cleaned between patients and at the end of every work day.
